Free AI Medical Bill Checker & Claim Denial Appeal Writer

Got a confusing medical bill or an insurance denial? ClaimClarity compares your bill with your insurer’s Explanation of Benefits (EOB), flags amounts worth questioning, and helps you write a billing inquiry or an appeal letter. Its AI case review uses Claude, an AI model from Anthropic, to read your documents together and explain what they show, quoting the exact lines it relied on. It’s free and needs no account.

What it does

  • Reads your documents: text PDFs, scanned PDFs, photos, text files and CSV charge lists. Scans are read on your own device with built-in text recognition.
  • Checks the numbers: compares your bill’s balance with what the EOB says you owe after payments, and flags possible repeated charges. Every flag is a question to verify, not proof of an error.
  • AI case review: Claude reads the bill, EOB, denial letter and any policy or clinician letter together. It explains the stated reason for a denial, what evidence is present, what is missing, and which questions to ask.
  • Ask about your case: type a question, and Claude answers from your documents only.
  • Drafts your letter: a provider billing inquiry, an insurance appeal, a request for your claim file, a request for a clinician’s letter, a financial-assistance request or a status follow-up. You edit everything before sending.
  • Keeps you on track: a deadline tracker with calendar reminders, call notes, and save/resume for your case.

How the AI works, and its limits

  • You see everything first. Before any text goes to the AI, the tool shows you exactly what will be sent. You can remove names, account numbers and anything else, and you must tick a consent box.
  • Grounded in your paperwork. Claude is instructed to use only your documents, never to invent diagnoses, policy terms, deadlines or legal arguments, and to quote the lines it relies on. The tool checks every quote against your original documents and drops any finding whose quote doesn’t match.
  • Checks the documents first. Unrelated files (a random PDF, a template, the wrong document type) are blocked before anything is sent, so the AI can’t produce a personalized letter from the wrong paperwork.
  • Math stays exact. Balances are calculated by the tool in whole cents, not estimated by the AI.
  • Fair-use limits. AI reviews are limited per visitor and per day so the service stays free. The rest of the tool works without limits.

What it can’t do

ClaimClarity is an organizing and drafting aid, not legal, medical or insurance advice. It does not decide whether a claim should be paid, whether care was medically necessary, or whether a law applies to you. It does not contact your insurer or provider or submit anything for you. Always follow the deadline and instructions on your actual notice, and check every statement before you send a letter. It is built for the US health insurance system; Medicare and Medicaid have their own appeal processes.

Privacy

The bill checks, document reading and letter templates run in your browser, and the tool doesn’t save your case unless you choose to save it. If you use the AI review, the text you approve is sent to One Apple Fall’s server and then to Anthropic for processing. Under Anthropic’s commercial terms, API inputs are not used to train its models, and standard retention is 30 days. One Apple Fall does not store your case text. The tool page has no ads or analytics scripts. This tool makes no HIPAA-compliance guarantee, so remove details you don’t want to share. See our privacy policy.

Questions

Is it free?

Yes, with no sign-up. AI reviews have fair-use limits per visitor and per day; all other features are unlimited.

Will it find money I’m owed?

It finds differences and questions worth checking, such as a bill that’s higher than what your EOB says you owe. A difference is not proof of overbilling, and the tool never promises savings or a successful appeal.

Which AI does it use?

Claude, made by Anthropic. The AI is used only after you review and approve the text that will be sent.

Does it work on a phone?

Yes. You can photograph a bill or letter and the tool reads it on your phone. A computer is easier for editing long letters.
