AI document review
Claude AI can read your bill, EOB, denial letter and evidence together, explain the denial reason, and point out what is missing.
You preview and redact the outgoing text first. AI findings need verification. No letters or documents are sent to an insurer.
Uses the documents you approve for sending. It does not search clinical literature or contact your insurer.
Start with the paperwork.
Upload a document or photo, or paste text. Review extracted text before using it.
Put the numbers in context.
Blank means unknown. Enter zero only when the document says zero.
Look a little closer.
Optional: compare line items for possible repetition and total mismatches.

Here’s what needs a closer look.
Based on the values you entered. Potential issues are questions to verify, not established errors.
A stronger response starts with facts.
Create a provider billing inquiry or an insurer appeal. Review every statement before sending.
AI is not connected. Guided case planning, local document reading and draft templates are available.
Replace [bracketed fields], remove unsupported statements, attach the actual evidence, and check the recipient and required plan forms. Drafting does not submit an appeal.
Turn clarity into a next step.
Keep copies, use the submission instructions on your notice, and record what happens.
Before you send
Deadline & routing
Calendar reminders do not file an appeal or extend a deadline.
Submission & follow-up
Call the billing office
“I’m reviewing claim [claim reference]. Could you send an itemized bill and payment ledger, confirm the latest EOB, and explain the entries I’ve flagged? Can you place the disputed amount on hold while you review it, and confirm that in writing?”

Guidance from the source.
US rules vary by coverage and circumstances. Guidance checked October 19, 2026.
What can this checker actually find?
It compares the bill balance against the EOB responsibility minus your recorded payments, after you confirm the documents match. It can flag repeated entered line items and arithmetic differences. It cannot validate coding, medical necessity, negotiated rates, insurer payment calculations or complete insurance coverage. It does not know whether a repeated charge is legitimate.
Is this appropriate for Europe, the UK, Medicare or Medicaid?
The bill arithmetic can help organize information, but this tool’s appeal guidance focuses on US private health coverage. Medicare has its own appeal levels and deadlines. Medicaid processes depend on your state and coverage. UK and European healthcare complaints require different rules; this tool does not apply US rights to them.
What if the denial is urgent?
If waiting for an ordinary review may seriously jeopardize your health or ability to regain function, contact your clinician and insurer immediately about an expedited appeal. Do not wait for this letter. Ask your plan about expedited external review and its submission requirements. This tool does not assess clinical urgency.
Should I stop paying while I dispute?
A dispute letter does not automatically pause payment, collections or insurance deadlines. Ask for a written hold on the disputed amount and confirmation of what is due while the matter is reviewed. Ask about financial assistance if affordability is the issue.
